The movie ended with Darkhold’s impact on Strange when a third eye opened on his forehead.

During their fight to grab the Book Of Vishanti, Wanda managed to grab America Chavez. With the help of her powers, she sent Strange and Christine to a universe destroyed. During their time in this universe in ruins, Strange came across a darker variant of his. This variant had used the Darkhold to such an extent that he ended up destroying his own universe.
Apart from having Darkhold around his waist, and being corrupted by it, one of the intriguing things about this Strange was probably his third eye. Although MCU’s Doctor Strange managed to beat him in combat, the question remained what this third eye was capable of.
Even MCU’s Doctor Strange opted to use Darkhold and dream walk in order to save America Chavez. In doing so, the ending saw Strange receiving a similar third eye. A skill he opted to use when Clea, played by Charlize Theron appeared in the mid-credits scene and asked him to tag along to save the multiverse after his travel caused a number of incursions.
For now, we can say that the third eye is the result of using Darkhold. Something that might have corrupted Strange not entirely but to some extent. The Third Eye Explained
The Third Eye of Doctor Strange does come from the use of Darkhold. But it doesn’t hold the power of making someone evil. According to the comics, the third eye is a manifestation of the Eye of Agamotto, one of the members of Vishanti. We saw Eye Of Agamotto first in Doctor Strange’s first film where it was used to contain the Time Stone. We later watched it being destroyed by Thanos in Avengers: Infinity War. Only to reappear in Spider-Man: No Way Home.
Mostly, the powers of the third eye work with someone who has a pure heart. So it’s far from being evil. The Dark Doctor Strange might have managed to gain that through Darkhold but he didn’t really put it to use. In MCU’s Doctor Strange’s case, he is of pure heart and the post-credits scene sees him ready to use it.
The Third Eye gives individuals the power to look through someone’s soul and even spot hallucinations. Apart from that, the eye also allows them to reenact certain events and in some cases release energy and open up portals.
