The woman, played by Charlize Theron, lets Strange know about the incursions caused by his travel into the multiverse. She asks him if he is ready to fix this. Strange answers the question by opening his third eye, which states that the event takes place sometime in the future. Strange might have spent time trying to excel in this new power.

Earlier in the movie, Doctor Strange met a darker version of himself in a universe destroyed because of this version’s incursions. The darker version crossed the limits when it comes to using the Darkhold. All of this for Christine. But in the same process, he ended up destroying his own universe. He also had a similar third eye on his forehead.

Even Stephen Strange used the Darkhold in order to dream-walk into his own universe to save America Chavez. This might be the reason Stephen Strange has the power of a third eye from now on. The woman and Doctor Strange walk into another universe that looks more or less like Dormammu’s Dark Dimension to close the scene.
