Though the story says it is because of his condition, there might be something else going on between the scenes, too.
And this behind-the-scenes has been going for quite a while in the series once the villains of the story were introduced. Earlier in the series, it was all about the male and female lead being newlyweds and learning about each other. That was the honeymoon phase of the series and the one that hooked the readers when the series started.
However, that felt like ages ago as the manhwa broke its promise of a wholesome plot. Now, the manhwa goes down the cliched path of misunderstanding and resentment from which none may recover. The female lead let out her frustration and wanted to know if she could do anything to appease her husband.
His tutor suggested that she should distance herself from him so the male lead could calm down. Distance is one of the key aspects of a good relationship but the female lead wanted to be in touch with him in some way. The tutor suggested that she should write a letter in that stead and convey her thoughts of him.
With a newfound vigor, the female lead got to working on that letter like a child with her new toy. The letter, however, was a mess since her handwriting was indiscernible from anything the tutor had seen. Though, it was in good faith so that her intent would be able to reach her husband just right. Or so it should have been.

At this, it is not even surprising to see the characters go through the problems they face, the lack of communication is mottling. But, the drama is what makes this romance interesting now, the wholesome vibes are never coming back once the series starts selling on Drama.
It would be nice for once if the female lead just gave up. She has a tutor that fancies her and would like to be with her. He does many things for her and would be there for a while, not burdened by the responsibilities of the crown. Meanwhile, the king just gets into a relationship with someone else, since he can just be with anybody and he doesn’t seem that affected.
