Ganta has known Isaki for a long time, and he knows that she is headstrong and hates to be pitied. And the reason she hates to be pitied is because everyone found her to be a poor thing due to her disease. She made it clear to Ganta long ago that she doesn’t want to be thought of that way.
But when our male lead saw her in a wheelchair and with oxygen stuck to her nose, he felt sorry for her. And he confessed that he felt for her that way despite knowing she doesn’t like to be thought of as such. Isaki was mildly furious at this and even teased him for acting in that regard.
