Unless you have an old API or get a high amount of reports, you don’t have anything to worry about. You can get reported for having an offensive trainer or Pokemon nickname, but that won’t get you banned. You will just be forced to change the name to something a little more appropriate.

To get permanently banned from the game, you would have to be caught actually cheating. While Niantic has programs to watch for suspicious behavior, players still report it themselves. After your account has been verified for spoofing and such, you will no longer be able to log into Pokemon Go.
The programs used for this are highly used and don’t make mistakes. If you have been permanently ban, and claim you haven’t cheated, Niantic won’t lift the ban. This is because it’s easy to say you didn’t do something, even though programs can prove that it’s been done. The captcha on the other hand. All it does is locks the game until you fill out the request.
